An officer in charge of a warship failed to check on a colleague whom she knew was lying on the floor - and discovered her dead from a diabetic coma seven hours later, an inquest has been told.

Lieutenant Helen Louise Voke was told when she took over the management of HMS Cornwall on Sunday, October 3, 2004, that Lieutenant Emma Douglas, pictured, was 'lying on the floor'.
